I will surely miss David. As a 17 year old kid he was like a second "Dad" to me. In recent years he was a patient and friend. I always look forward to seeing him on the schedule. We often laughed about the "good old days" on the Cook block. The world is a better place because of him...he was truly one of the "good guys".

Dearest Family of David and Addie, I always called your mom and dad "lovebirds", "sweethearts" and other terms of endearment. They were so much in love... Both were so very special to me. I loved them. Their sweet, beautiful, and smiling faces will be thought of often and will always be close in memory. May the burden of your hearts be lifted by the knowledge that they are once again holding hands in the presence of God. Love and sympathy, Carol Gwynn
